Where to Find All Potions in Halls of Torment

The community wiki lists six potion types and fifteen ingredients, with hall locations below. “40 potions” refers to the Brewmaster requirement to drink 40 health potions in one run, rather than forty ingredient types. Ingredient details are a single wiki source.
Six potions, not forty materials
The Fandom potion page lists six potions. Strong Wine is available first. The other five open as you collect ingredients, after Cupbearer is unlocked. “40 potions” is the Brewmaster quest: drink 40 health potions in a single run. It is not a count of ingredients. Trail to Decay asks you to collect 11 potion ingredients. Both quest notes are from the wiki quest table.

Fifteen ingredients and their halls
Fifteen ingredients. Each sits in one hall, two normal spots and one Agony spot, at a random position. Picking one removes it from that hall permanently. Gatherer's Charm helps you locate the spot. The wiki does not publish x/y coordinates, so this table stops at the hall.

Questions and answers
Do potion ingredients respawn every run?
Not on the wiki. A collected ingredient is removed from that hall. Potions themselves refill up to the listed capacity each run.
Which potions did the DLCs add?
The potion page’s six recipes are the base set. I do not have a second source that adds a seventh recipe in The Boglands or The Lost Archives, so I am not inventing one.
